Submitting a form does not create an attorney-client or doctor-patient relationship.

2. SMS Terms & Conditions
2.1 SMS Consent Communication
Information (Phone Numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes. SMS opt-in or phone numbers for the purpose of SMS are not being shared with any third party and affiliate company for marketing purposes.
2.2 Types of SMS Communications
If consent has been given to receive text messages from Medrek Group, messages may be related to the following:
- Request services
- Follow-up messages
Example: [Medrek Group] Thank you for submitting your request with Medrek Group. We look forward to seeing you on August 12 at 2:00 PM. Reply HELP for assistance or STOP to opt out.
2.3 Message Frequency
Message frequency may vary depending on the type of communication. For example, up to 2 SMS messages per week may be received related to your request.
2.4 Potential Fees for SMS Messaging
Standard message and data rates may apply, depending on the carrier's pricing plan. These fees may vary if the message is sent domestically or internationally.
2.5 Opt-In Method
Opt-in to receive SMS messages from Medrek Group can be done by filling out a website form (including the intake or contact form) and checking the SMS consent box.
2.6 Opt-Out Method
Opting out of receiving SMS messages can be done at any time by replying STOP to any SMS message received. Alternatively, you may contact us directly to request removal from the messaging list.

If SMS messages are not desired, the SMS consent box on forms can be left unchecked. Consent to receive SMS is not a condition of receiving free Medrek Group intake or other Medrek Group support.

3. Intake and optional attorney introduction
Completing a Medrek Group intake authorizes Medrek Group to contact you about your inquiry. It does not authorize Medrek Group to send your information to a law firm. If you later request attorney assistance, Medrek Group may show disclosed independent participating law firms. You choose whether and where your information is sent. Before any transfer, Medrek Group must obtain a separate authorization naming the specific firm. A law firm—not Medrek Group—decides whether to offer a consultation or representation. No attorney-client relationship exists unless you enter into an agreement directly with that firm.
Participating law firms may pay Medrek Group fixed fees for documented multilingual intake and administrative services. Fees are not based on whether you retain a firm, the amount recovered, or the legal fees earned. You do not pay Medrek Group for the intake or attorney-choice service. Participation does not guarantee that a firm will accept your matter or achieve any result.
4. Call recording
Calls may be recorded and transcribed for intake documentation, quality, and compliance when notice is given and consent is obtained. You may decline recording.
5. Acceptable use
You agree to provide accurate information, use our services lawfully, and not attempt to disrupt or misuse the website, forms, or messaging program.
6. No professional advice
Content on this site and in our messages is for general informational and administrative support only. For legal or medical advice, consult a licensed professional. Educational articles do not create an attorney-client relationship and are not a substitute for advice about your situation.
7. Disclaimer of warranties
Services are provided "as is" and "as available." To the fullest extent permitted by law, Medrek Group disclaims warranties of merchantability, fitness for a particular purpose, and non-infringement. Service availability varies by state.
8. Limitation of liability
